What is the Role of a Geriatric Care Manager?

At Elder Resources, we develop flexible, personalized care management plans.
On a case-by-case basis, Elder Resources:

Our assessment process begins with an initial consultation with family members, either in person or by telephone. We gather essential data including a full family and medical history prior to meeting with the elder. One of our associates will then visit the elder at home, in a facility, or at a family member's house to conduct an in-depth evaluation. This process includes:

At Elder Resources, we are committed to honoring our client's wishes to remain in their own home with independence and dignity. We can arrange for full or part-time home care services including home health aides, companions, and rehabilitation staff. Elder Resources also provides a link to other valuable services including meal delivery, escorted transportation, bill payment, daily money management, and adult day health programs.
Elder Resources is not affiliated with any of the services we recommend. We are an independent company, providing families with the most professional objective recommendations based on our client's needs and his or her expressed preferences.

Elder Resources offers recommendations regarding alternative housing options including retirement communities, assisted living, and nursing homes. Our recommendations are based on our client's physical and emotional needs, as well as a thorough analysis of his or her finances. Once a decision to move is reached, Elder Resources can assist with all the details involved in this transition, including arranging for movers, organizing the clean-out of the house or apartment, and providing support to ease the transition. Elder Resources regularly visits all recommended alternative living facilities to ensure that high quality care is being provided.

Elder Resources provides referrals to other carefully selected professionals including elder law attorneys, financial planners, and physicians. For clients experiencing memory problems, Elder Resources makes referrals to local memory disorder clinics for diagnostic evaluations.

Elder Resources provides ongoing care management services to attorneys or family members acting as guardian for an elder. These services include arranging for necessary services, ongoing monitoring of care needs, assistance with transitions to nursing homes or assisted living communities, and providing regular written status reports

Elder Resources provides a vital link between our clients, their family members, and the medical, legal, and social service professionals involved in providing care. We act as "eyes and ears" for families, particularly those living at a distance, and provide regular status updates via phone, e-mail, or in person.
